Rights & Responsibilities

1. To be treated with consideration, respect, and full recognition of your dignity and individuality, including privacy in the treatment and care of your personal needs.
2. To be fully informed of your medical condition, participate in care planning, and make choices about your treatment unless adjudged incompetent or otherwise found to be incapacitated under the laws of the State.
3. To be fully informed in advance about care and treatment, as well as any changes that may affect your well-being.
4. To choose activities, schedules, and healthcare that are consistent with your interests, assessments, and plans of care.
5. To be fully informed, before or at the time of admission and throughout your stay, of services available in the facility and related charges, including any charges for services not covered by Medicare or not included in the facility’s basic daily rate.
6. To associate and communicate privately with persons of your choice and to send or receive your personal mail unopened.
7. To exercise your rights as a resident of Lake Butler Hospital and as a citizen or resident of the U.S.
8. To be free from interference, coercion, discrimination, or reprisal from Lake Butler Hospital when exercising your rights.
9. To request and receive a copy of your healthcare record.
10. To be fully informed in a language you understand of your total health status, including, but not limited to, your medical condition.
11. To refuse treatment and to decline participation in experimental research.
12. To create an Advance Directive, such as a Power of Attorney for Healthcare, that directs life-sustaining treatment decisions.
13. To manage your personal financial affairs free from interference.
14. To choose a personal attending physician or other healthcare services at Lake Butler Hospital, or allow the hospital to make appropriate appointments. Your physician must have practice privileges at Lake Butler Hospital.
15. To personal privacy and confidentiality, including in accommodations, medical treatment, written and telephone communications, personal care, visits, and meetings of family and resident groups.
16. To be assured of confidential treatment of your personal and medical records and to approve or refuse their release to any individual outside of the facility, except as required by law or a third-party payment contract.
17. To unrestricted communications with public officials and attorneys.
18. To privacy during visits with family, friends, clergy, or social workers.
19. To have reasonable access to telephones to make and receive confidential calls.
20. To participate in social, religious, and community activities that do not interfere with the rights of other residents.
21. To voice grievances without discrimination or reprisal.
22. To not be required to perform services for the hospital unless they are included for therapeutic purposes in your care plan.
23. To retain and use your personal clothing and possessions as space permits, unless doing so would infringe on the rights of other patients or be medically contraindicated, as documented by your physician in your medical record.
24. To privacy during visits with your spouse. If both you and your spouse are residents in the Short Term Rehab program, you may share a room unless medically contraindicated, as documented in your record.
25. To have physical and emotional privacy in treatment, living arrangements, and personal care.
26. To access transportation services. Case Management can assist with coordinating transportation, though these services may not be covered by your insurance.
27. To be free from verbal, sexual, physical, chemical, and mental abuse, corporal punishment, and involuntary seclusion.
28. To be free from physical or chemical restraints unless authorized in writing by a physician for a specific and limited period. Restraints will be used only to protect you from injury to yourself or others.
29. To have any real or alleged abuse reported immediately to administration in accordance with regulations.
30. To be transferred or discharged only for medical reasons, for your welfare or the welfare of other patients, or for nonpayment of your stay (except as prohibited by Medicare and Medicaid).
31. To receive reasonable advance notice to ensure your orderly transfer or discharge, so you may make your wishes known and participate in the planning of your move.

Resident Responsibilities

As a Short Term Rehab Program resident, your responsibilities include:

1. To provide, to the best of your knowledge, accurate and complete information about your present illnesses, past illnesses, hospitalizations, medications, mobility, and other matters related to your health.
2. To report unexpected changes in your condition to the healthcare team.
3. To indicate whether you clearly understand your plan of care or require further explanation.
4. To actively participate in your plan of care. Refusal of treatments or therapies may result in discharge from the Short Term Rehab program.
5. To ensure that the financial obligations of your healthcare are fulfilled as promptly as possible.
6. To follow hospital rules and regulations affecting your care and conduct.
7. To be considerate of the rights of other patients and hospital staff.
8. To be respectful of the property of other persons and the hospital.
9. To follow Lake Butler Hospital’s smoke-free campus policy.
