Do you need outpatient therapy services? Remember, you always have a choice where you receive therapy. There is no need to travel out of town. Let your provider or surgeon know you would like to receive therapy at Lake Butler Hospital. Your provider will write the prescription for the therapy, and you can call 386-496-9277 to set up an appointment.
